  1. Published an updated version of form CRU1 Register a claim for compensation.

  2. Updated the Compensation Recovery Unit address in the following forms: CRU1G, CRU1J, CRU1-REP, CRU1-SOL.

  3. Published an updated version of form CRU1 Register a claim for compensation.

  4. Replaced all forms except CRU1503 with new versions. Removed order form for CRU1 form as this service no longer exists.

  5. Replaced CRU1G form - Register a claim for compensation, with revised version.

  6. Replaced CRU1503 form: NHS charges, contributory negligence - request liability review, with revised version.

  7. Replaced CRU 1 form with 2 page printable version.

  8. Replaced CRU forms with amended versions.

  9. Replaced CRU forms with updated versions (unlocked to allow completion in word processing software).

  10. Published updated forms: Register a claim for compensation, Solicitor taking over a registered case, Solicitor returning responsibility for a case to the insurer. DX postal address removed from all forms.

  11. First published.
