These forms are for local authority use only.

Local authorities and Health and Social Services Trusts need to send a monthly return to HMRC about certain children they look after. The return should include form CTF15 and an individual form CTF15 (Child) for each child.

Email HMRC to ask for this form in Welsh (Cymraeg).
