You don’t need to use this form if you:

  • already get Carer’s Allowance
  • get Child Benefit for a child under 12
  • are a foster carer and get National Insurance credits from HM Revenue and Customs

The carer or their representative must fill in this form, not the person being looked after. They must print the form and fill it in with a pen.
