This pack provides 3 resources for NHS breast screening services to send to their local GP practices 6 weeks before their patients are due to be invited for breast screening.

The 2 open document text resources are for services to add their own information before sending to the GP practice. The Excel spreadsheet is for data collection from the practice.

The resources are:

  • a template email for breast screening services to send to GPs to request patient information
  • a form for GP practices to complete when providing patient information requested by their breast screening service
  • a template document to provide GP practices with information about their local breast screening service and about breast screening
