Blood products: model health certificates
Competent authorities of exporting countries should use the relevant model health certificate as a template to create a version exporters can apply for to export blood products to Great Britain, the Channel Islands or Isle of Man.

Blood products exported to Great Britain must be accompanied by the relevant health certificate. Exporters should read the guide on how to complete a health certificate.
Certificates GBHC094X and GBHC501
Non-EU countries must use certificate GBHC95X from 1 January 2021.
Non-EU countries must use certificate GBHC501 from 30 April 2024.
EU countries must use certificate GBHC501 from 31 January 2024.
Model health certificate for blood and blood products from equidae for purposes outside the feed chain (BP-E).
Certificates GBHC095X and GBHC500
Non-EU countries must use certificate GBHC095X from 1 January 2021.
Non-EU countries must use certificate GBHC500 from 30 April 2024.
EU countries must use certificate GBHC500 from 31 January 2024.
Model health certificate for blood products not intended for human consumption that could be used as feed material (BP).
Certificates GBHC096X and GBHC502
Non-EU countries must use certificate GBHC096X from 1 January 2021.
Non-EU countries must use certificate GBHC502 from 30 April 2024.
EU countries must use certificate GBHC502 from 31 January 2024.
Model health certificate for untreated blood products, excluding those of equidae, for the manufacture of derived products for purposes outside the feed chain for farmed animals (BP-U).
Certificates GBHC097X and GBHC503
Non-EU countries must use certificate GBHC097X from 1 January 2021.
Non-EU countries must use certificate GBHC503 from 30 April 2024.
EU countries must use certificate GBHC503 from 31 January 2024.
Model health certificate for treated blood products, excluding those of equidae, for the manufacture of derived products for purposes outside the feed chain for farmed animals (BP-T).
Certificate GBHC200
Use this certificate from 21 June 2021.
Model health certificate for processed blood and blood products (excluding serum from equidae) for uses outside the feed chain from New Zealand.

We have removed all EU certificates ending in E for blood products. We are currently in the process of replacing these with new certificates that can be used by all countries. EU countries must use the new certificates from 31 October 2023. Non-EU countries can continue using certificates ending in X or start using the new certificates. We will communicate the deadline for non-EU countries switching to the new certificates in due course.
