You should fill in this part of the form B6.5 if your intended activity is limited to discharging treated domestic sewage effluent of up to 15 cubic metres (15m3) a day to ground or up to 20 cubic metres (20m3) a day to surface water, see the form guidance document for assistance whist completing your form.
