     Minor amends to specialist outcomes assessment section and referral alerts section to improve clarity.

  2. 24 February 2025

     Update to guidance on process for completing a screening incident assessment form (SIAF). Services should now only complete if the actions of services are a contributing factor, such as delayed referral from screening to vascular services.

  3. 1 October 2024

     Added clarification that publication was previously published by Public Health England.

  4. 12 April 2019

     Minor changes to the report guidance, including removal of 2 specialist assessment outcomes that are no longer used.
