TCM1000433 - How to make sure form TC241 is fully completed

Step 1

On the TC241 ‘Request to replace a missing payment’ form

  • all fields on questions 1 to 3 under the ‘Your details’ section should be fully completed
  • all fields on questions 4 to 13 under the ‘Payment details’ section should be fully completed

Note: The customer must provide the due date and amount of each missing payment.

Note: The customer must provide the date and amount of the last payment they received.

  • the ‘Declaration’ field should be completed with the

    • payee signature
    • payee name
    • date
    • daytime phone number
    • daytime fax number, if applicable.
