How to complete the PAD

Version 2.0

Changes to previous version - amended guidance

Step 1

If you are in an open record, go to .

If you are not in an open record, go to .

Step 2

On the ‘CPT’ desktop screen

  • select the ‘POca Control Panel’
  • select ‘View Worklist’. You will be taken to the ‘View Worklist’ screen
  • select the record you are working on
  • select ‘Open New PAD’

  • go to .

Step 3

In Microsoft Excel

  • select ‘SEES’ on the toolbar
  • select ‘xmls Forms’ from the options available
  • select ‘National’
  • select ‘POca Request’ from the options available
  • select ‘OK’. You will be taken to the ‘PAD’ main screen

If the customer has an appointee, go to .

If the customer does not have an appointee, go to .

Step 5

Complete the fields shown

Note: Select the CAPS lock on your keyboard to make sure you complete the PAD in uppercase.

  • select the ‘Select if customer consent received’ checkbox

Note: If you do not have the customer’s consent you must not proceed or attempt to complete and send the PAD.

  • select the ‘Select if the account is for an appointee’ checkbox
  • enter the appointee’s title in the ‘Title’ field
  • enter the appointee’s first name in the ‘First Name’ field
  • enter the appointee’s middle initial in the ‘Middle Initial’ field
  • enter the appointee’s last name in the ‘Last Name’ field
  • select the ‘Accommodation Type’ radio button

Note: You should select the ‘Other’ radio button if the place where the customer lives is not in a house, a block of flats or a flat in a house, for example they live in a caravan park or a houseboat.

  • enter the house, flat or business street number, if applicable in the ‘Number for house, flat or business’ field
  • enter the suffix, if applicable in the ‘Suffix’ field

Note: You should complete this field if the customer lives at a number where there is a suffix used to complete the house, flat or business number, for example Flat 2B.

  • enter the name of the house, flat or business, if applicable in the ‘Name for house, flat or business’ field
  • enter the area or district, for example Deepdale, Walton-le-Dale, Ashton in the ‘Area/District’ field
  • enter the town in the ‘Town’ field
  • enter the county in the ‘County’ field
  • enter the postcode in the ‘Postcode’ field
  • enter the appointee’s NINO in the ‘National Insurance Number’ field
  • enter the appointee’s date of birth in the ‘Date of Birth’ field

Note: You must enter the date of birth in the format of DD/MM/YYYY.

  • select the applicable ‘Language’ radio button

Note: The ‘Language’ radio button is defaulted to ‘English’.

  • select the applicable Requested Media’ radio button

Note: The ‘Requested Media’ radio button is defaulted to ‘’Standard Print’.

  • select if the POca is to be used for TCO or Child Benefit

Note: The TCO and Child Benefit checkboxes should both be selected if the request is for both.

Note: You must enter a Child Benefit reference number if you have selected the POca be used for CBO only or both TCO and CBO.

  • enter the customer’s NINO in the ‘National Insurance Number’ field
  • enter the customer’s Child Benefit reference number in the ‘Customer’s Child Benefit Number’ field
  • enter the customer’s full name in the ‘Customer’s Full Name’ field

Note: An error message will display if you do not complete a mandatory field or if the entry you have made is not recognised.

  • select ‘Send Form’

Note: Selecting ‘Send Form’ completes the action and sends the PAD referral.

Complete the fields shown

Note: There are 35 characters available in the ‘Surname’ field.

  • select the ‘Accommodation Type’ radio button

Note: You should select the ‘Other’ radio button if the place where the customer lives is not in a house, a block of flats or a flat in a house, for example they live in a caravan park or a houseboat.

  • enter the house, flat or business street number, if applicable in the ‘Number for house, flat, business’ field
  • enter the suffix, if applicable in the ‘Suffix’ field

Note: You should complete this field if the customer lives at a number where there is a suffix used to complete the house, flat or business number, for example Flat 2B.

  • enter the name of the house, flat or business if applicable
  • enter the area or district, for example Deepdale, Walton-le-Dale, Ashton in the ‘Area/District’ field
  • enter the town in the ‘Town’ field
  • enter the county in the ‘County’ field
  • enter the postcode in the ‘Postcode’ field
  • select if the customer is under 16 years of age

Note: You must enter a Child Benefit reference number if you have selected ‘Customer is under the age of 16’.

  • enter the customer’s NINO in the ‘National Insurance Number’ field

Note: You must enter the full NINO including the last alpha character.

  • enter the customer’s date of birth in the ‘Date of Birth’ field

Note: You must enter the date of birth in the format of DD/MM/YYYY.

  • select the applicable ‘Language’ radio button

Note: The ‘Language’ radio button is defaulted to ‘English’.

  • select the applicable ‘Requested Media’ radio button

Note: The ‘Requested Media’ radio button is defaulted to ‘Standard Print’.

  • select if the POca to be used for TCO or Child Benefit

Note: The TCO and Child Benefit checkboxes should both be selected if the request is for both.

Note: You must enter a Child Benefit reference number if you have selected the POca be used for CBO only or both TCO and CBO.

Note: An error message will display if you do not complete a mandatory field or if the entry you have made is not recognised.

  • select ‘Send Form’

Note: Selecting ‘Send Form’ completes the action and sends the PAD referral.

Step 7

If you do not need to complete another PAD

  • select ‘Yes’
  • take no further action.

If you do need to complete another PAD, select ‘No’. A new PAD will be opened for you to complete.
