How to request a re-issue of the IYS17

This form must be completed by Priority 1 for joint claims where it has been identified that a date of death reported is earlier than the tax credits end date and the surviving partner has made a claim to Universal Credit.

This will result in a change to the assessment period used to in year finalise the claim and in cases where the IYS17 has already been issued, an amended IYS17 will need to be re-issued showing the correct assessment period.

Step 1 {#}

Complete the Request re-issue of IYS17 template with the following information

  • deceased name
  • deceased NINO
  • deceased date of birth (DoB)
  • date of death (DoD)
  • tax credits end date
  • go to Step 2

Send the completed template via email to:

With the subject heading: Priority 1 – DoD Prior to TC End Date - Re-issue IYS17

  • take a copy of the template and keep with the case papers
  • return to the guidance you were following
