Form to provide cost recovery details for your application to the Building Safety Fund, Private Sector (ACM) Cladding Remediation Fund, or Social Sector (ACM) Cladding Remediation Fund.

Who should use this form

Use this form to provide cost recovery details for applications to the following funds:

  • Building Safety Fund
  • Private Sector (ACM) Cladding Remediation Fund
  • Social Sector (ACM) Cladding Remediation Fund

Do not use this form if you are applying to the Cladding Safety Scheme. Please contact your caseworker on how to submit cost recovery details.

What you need to complete the form

To complete the form, you need to have:

  • the date the cladding was installed on your building
  • details of who installed the cladding
  • details of warranty provider
  • details of insurers
  • details of any cost recovery action you have take to date

We highly recommend you talk to your delivery partner or client side support provider (CCSP) about your responses before you submit this form.

There are 2 parts to the form:

  • cost recovery questionnaire
  • conflict of interest statement

Applicants must complete both parts, to fulfil obligations under the Grant Funding Agreement.
