Details

NHS Continuing Healthcare (CHC) is a package of care (outside hospital) arranged and funded solely by the NHS where the individual has been found to have a ‘primary health need’ as set out in the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care.

Such care is provided to an individual aged 18 or over, to meet needs that have arisen as a result of disability, accident, or illness.

NHS CHC can be provided in range of settings including a care home, hospice or a person’s own home.
