This form should be completed to apply for an inspection to approve premises which will operate as a control post to rest cattle, sheep, pigs, goats and horses (other than registered horses).
A sketch/plan showing the boundaries of the control post must be included with the form.
The form should be sent to your local APHA Office giving as much notice as possible, and not less than 14 days.
