Your test must be from a clinic approved by the Home Office. We will not accept a TB test certificate from a clinic we have not approved.

- valid identification which contains your photograph such as a original passport, national ID card or Refugee Asylum Seeker Card
- colour copy of identification document such as passport and national ID card
- 3 recent passport size photographs with white background, neutral face expression and without glasses
- completed applicant history questionnaire - this can be completed online or downloaded from www.amcpk.com
- the TB test fee
- confirmation email of your appointment from AMC
