If you are an employee and need to claim Statutory Sick Pay (SSP) from your employer use form SC2.
Use form SC2 if you are an employee, you’ve been sick for 4 or more days in a row and need to claim SSP from your employer.
When you’ve filled in the form give it to your employer, it will help them decide if you can get SSP. Keep a copy for your own information.

You’ll need to fill in the form fully before you can print it. You can’t save a partly completed form so gather all your information together before you begin to fill it in.
