You should calculate the amount of advanced funding in weeks, regardless of how often the employee is paid.

Email HMRC to ask for this form in Welsh (Cymraeg).

If you’re an employer

Use the the online service to apply for an advance of statutory maternity, paternity, adoption or shared parental pay.

You’ll need the sign in details you use when you submit returns for Self Assessment, VAT or Corporation Tax.

If you’re an agent

You’ll need the sign in details you use when you sign in for:

• Agent Services Account
• Self Assessment
• VAT (you’ll also need to be VAT registered yourself)
• Corporation Tax
• Machine Gaming Duty
