This form is part of the guide to using animal by-products to make pet food.

Your premises, operator and equipment must be approved before they can be used for pet food production.

You must apply for approval if you make pet food in a domestic houses using meat fit for human consumption, or category 3 ABPs.

For customers in England, return the completed form to APHA Customer Service Centre Worcester.

For customers in Scotland and Wales, return the completed form to your local APHA Field Service Office.
