Use these notes to help you make different notifications, including entering or exiting the regime, wind down periods and breach notifications.

These notes will help you to complete the following different notifications:
- QAHC information returns
- entry and exit notifications
- breach and cure notifications
- breach and exit notifications
- ownership condition notifications, including:
- a breach within the first 2 years
- no reasonable expectation of meeting this condition within 2 years
- wind-down period notifications, including where a QAHC then acquires assets or raises capital
