Appendix 3. Postal packaging instructions for flu kit samples

How to send flu kit parcels through the post

Collect the samples required using the materials provided

Label the samples with the patient’s details, including their first name and surname, date of birth and sample collection date

Complete the request forms. See note 1.

Ensure the patient details on the request form and the sample match.

Together with the absorbent material, place the swab sample container inside the rigid green-top transport tube. Make sure the lids are securely tightened on both tubes to prevent sample leakage. See Figure 1.

Place the rigid green-top transport tube inside the clear plastic biohazard bag that is attached to the respiratory investigation form, and seal using the peel off strip. See Figure 2.

Place the sample and attached respiratory investigation form inside the pre-paid tracked 24 white plastic envelope packaging (UN3373). Ensure the white envelope is sealed using the peel off strip. See Figure 3.

Send the package to UKHSA Manchester by Royal Mail.

Note 1
Make sure to include:
- patient details - first name and surname
- date of birth
- NHS number, if possible
- the sample collection date
- the location or address for the reports
- the ILog field with the number allocated to your outbreak - phone 0161 276 8843 to obtain an ILog number
