As part of the BBC Marmara report publication, this flyer was produced to summarise common themes from investigations where navigational safeguards had been circumvented. It serves to highlight a continuing trend of collision, grounding and contact accidents that share similar contributory factors involving lookouts and the Bridge Navigational Watch Alarm System (BNWAS).
