This form should be used by railway infrastructure managers, railway operators and maintainers whose staff or property is involved in an accident or incident.

Fill in this form to report incidents notifiable under Schedule 3 of the The Railways (Accident Investigation and Reporting) Regulations 2005 . Use the ‘Extract from guidance’ notes to find out what types of incident are notifiable and what information you should include.
