If you’re an intervention provider (IP) involved in delivering Prevent programmes, use this form to provide information about your caseloads.

The Home Office Interventions Team will use the information you submit to assess your capacity to take on additional cases.

All information will be securely collected and processed by the Interventions Team to assess caseload capacity.
