Details

Use this form to claim back:

• your probate, letters of administration, confirmation, or grant de bonis non application fee
• the cost of hiring a legal adviser to help with your probate, confirmation, letters of administration, or grant de bonis non application and cost of certifying a chain of representation in Northern Ireland (up to a maximum of £1,500)
• if a successful legal costs claim has previously been made by an estate, the reimbursement is capped at a total of £1,500 across all claims
• for example, if your legal fees for getting probate cost £1,000 for your first application, you are able to claim back a further £500 for any legal fees related to probate

If you need a printed or accessible version of the form, contact the Infected  Blood Support Scheme (IBSS) in the nation where the person who died was infected.

Before you start

You’ll need:

• the Application Reference Number from your interim compensation decision letter
• details of the legal representative you used
• the payment receipt for your probate or confirmation application fee
• itemised invoice for legal services from the legal representative
• copies of proof of payment for legal services, such as a receipt from your legal adviser

Where to send the form

Send your completed form and copies of relevant invoices to the IBSS who processed your claim for compensation.
