This form can be used when:

  • the company is registered in Northern Ireland
  • insolvency proceedings are concluded in the UK
  • the company is to be dissolved
  • dissolution consent has been given by all other member state liquidators

This form can only be used for insolvency proceedings which began before 31 December 2020.
