Request for number of reviews carried out by the NHS Trust Development Authority on trust serious incident policies regarding maternity departments and the findings from 2013 to date.

NHS Improvement held some of the information requested and decided to withhold all of the information it held under sections 31 and 41 of the Freedom of Information Act 2000.
