The request was for the number of documentation incidents reported to NRLS in the financial years 2013/14, 2014/15 and 2015/16 where the degree of harm was recorded as “death”. A brief summary of the incidents in 2014/15 and 2015/16 was requested.

NHS Improvement held the information requested and released it.
