Contracted service provider declaration
I confirm that the above named DCO has completed the required refresher training as stipulated in Annex C to perform duties as a DCO, in addition to any additional centre-specific training required for effective completion of the DCO role. There have been no reported changes in circumstances since the DCO has been on leave or they have already been reported to VCAT. I declare that the DCO is sufficiently trained and confident to resume duties as a DCO.
