Form(s) should be used by any responsible tri-Service military authority and MOD Contractors/Partners.
Form(s) should be used by Service personal, Service leavers, and their families and by third party organisations, namely, charities and Local Authorities who may be directly approached by Service Personnel and their families requiring support.

Both forms 1 and 2 will be received centrally by Veterans UK and triaged for response by either Veterans Welfare Service (in the case of clients with severe physical or psychological disablement or those considered as having an enduring welfare need with which they will require support post-service) or Defence Transition Services in all other cases.
