If you require a CA1 form where the court appointment order was made on or prior to the 31 December 2023 then please contact courtappointee@justice.gov.uk

The CA2024 form should be used for claims where the court appointment order has been made between the 1 January 2024 and 31 December 2024 only.

The CA2025 form should be used for claims where the court appointment order has been made on or after the 1 January 2025 only.

The CA2026 form should be used for claims where the court appointment order has been made on or after the 1 January 2026 only.

Providers should ensure claims are submitted within the applicable time limits. In some circumstances, late submissions may lead to a reduction or disallowance of costs in accordance with the relevant regulations
