This Department for Work and Pensions (DWP) report includes:

• establishment of new governance structure and oversight mechanisms and alignment with NHS best practice 
• an updated mandatory training programme for all clinicians to ensure consistent standards and accountability
• the implementation of risk management tools designed to identify, assess, and respond to incidents more effectively
• an addendum which outlines the Clinically Significant Event Analysis (CSEA) reporting for the Functional Assessment Service (FAS) contract at the Department for Work and Pensions (DWP) and across the DWP clinical team (published 1 June 2026)
