Clinical Excellence Award holders: employment change notification

Use this form to tell ACCEA about changes in your employment circumstances.



You must let the Advisory Committee on Clinical Excellence Awards (ACCEA) know of changes to your employment circumstances, including:

  • moving to another trust
  • changing the number of sessions you work
  • taking unpaid leave
  • retiring

Please complete the form and then send it back to the ACCEA Secretariat.
