Details

If a coroner investigation has or is taking place into the cause of death there is no requirement for the medical certificates to be completed however the coroner’s permission will be required for the cremation to take place and this will be provided through completion of this form, Cremation 6.

This form should be used for deaths occurring in England, Wales, Scotland, Northern Ireland, the Channel Islands or the Isle of Man where the cremation will take place in England or Wales, when relevant.
