Form for medical referees to authorise the cremation of a deceased person's body. This form should be used for deaths occurring in England or Wales, and where the cremation will take place in England or Wales.

Details

This form must be completed by medical referees.

The form should be used to confirm that:

  • the cremation meets the requirements set out in the Cremation (England and Wales) Regulations 2008
  • where a coroner is under a duty to investigate under section 1 of the Coroners and Justice Act 2009, an investigation has been opened

There is a different form if the death occurred in Scotland, Northern Ireland or the rest of the British Islands and the cremation will take place in England or Wales.
