The appointment can be made by order or under powers contained in an instrument.
If the appointment was made, or order was obtained, under powers in a registered charge created:
- before 6 April 2013, complete part A of the form
- on or after 6 April 2013, complete part B of the form
You will need to print this form at full size on white A4 sized paper.
