Use this form to apply to the Driver and Vehicle Standards Agency (DVSA) to become a DAS motorcycle instructor.

How to apply

  1. Download and save the application form on your computer or device before you start to fill it in.

  2. Open the form on your computer or device, fill it in and save it again.

  3. Email the filled in form to CBTDAS.assessments@dvsa.gov.uk
