Details

Use this form if you are an employer or a trade union where a voluntary agreement was reached after the union made a request, or an application to the CAC, for statutory recognition, and:

  • the parties have not agreed a method of bargaining

or

  • they have agreed a method of bargaining but one party alleges that the other has failed to carry out the terms of the agreement.
