Use this form to report these medical conditions:

  • attention deficit hyperactivity disorder (ADHD)
  • autistic spectrum disorders (ASD)
  • Tourette’s
  • severe communication disorders

Check the list of health conditions if yours is not listed.

Use a different form to report these conditions if you only have a lorry, bus or coach licence.
