At around 15:50 hrs on 1 June 2017, a person was struck and fatally injured by a train travelling from Cheltenham Spa to Maesteg, at Trenos footpath crossing, near Llanharan, Rhondda Cynon Taf, South Wales.

The crossing serves a footpath near Trenos Farm on a footpath linking the villages of Bryncae and Brynna, about 1.6 km (1 mile) west of Llanharan. Two railway tracks cross the footpath at this location.

Our investigation will seek to identify the sequence of events which led to the accident and will consider:

  • the actions of the person
  • the actions of the railway staff involved
  • any underlying management factors

Our investigation is independent of any investigation by the railway industry, the British Transport Police or by the industry’s regulator, the Office of Rail and Road.

We will publish our findings, including any recommendations to improve safety, at the conclusion of our investigation. This report will be available on our website.
