All providers are reminded that it is a contractual requirement to inform us if you lose or decide to replace one of your category supervisors.
If there has been a change in the identity of any of your supervisors and we haven’t been informed then you should notify your contract manager as soon as possible.
Supervisor self declaration forms will be routinely checked during visits from your contract manager to ensure the competence requirements are met.
