Car or motorcycle licence
You must tell DVLA if you have an atrial or ventricular defibrillator.
You can also fill in form DEFIB1 and send it to DVLA. The address is on the form.
Bus, coach or lorry licence
Fill in form VOCH1 and send it to DVLA. The address is on the form.
