Hazard
The product presents a high risk of injuries due to the green auto-tilt lever. This lever operates a hydraulic ram to raise and tilt the slide bed during deployment. When the Green Lever is released, the bed does not hold in the raised position and falls under its own weight back to the chassis. The tilt system is also not fitted with devices to prevent a gravity fall or uncontrolled movement in the event of hydraulic failure. A risk of serious injury to operators and other persons may arise from impact with the slide bed, or from crushing between the slide bed and vehicle chassis and other fixed structures such as storage lockers.
The machine did not satisfy the requirements of The Supply of Machinery (Safety) Regulations 2008.
Corrective action
Recall and withdrawal of the product.
The manufacturer has voluntarily suspended further sale of recovery vehicles featuring the Green Lever auto-tilt function.
The manufacturer has also voluntarily initiated a recall of recovery vehicles featuring the Green Lever auto-tilt function from end users. The auto-tilt function will be permanently disabled and hydraulic burst valves will be installed. The manufacturer has provided instruction to users on how to take immediate action to remove the Green Lever as an interim measure. Users should contact J & J Conversions Limited for advice and to make arrangements for the modifications to be carried out.
