On 24 August 2001, a crewman sustained an injury during the firing of cannon on sailing vessel Grand Turk while alongside at Portsmouth.

debris from the previous firing was disturbed by the wrong sequence of swabbing and worming, which caused a premature ignition

Recommendations have been made aimed at reducing the risk of similar accidents occurring in the future if implemented.
