Crew member from Atlantic D fell from mast and injured spine

Action taken

The Chief Inspector of Marine Accidents made recommendations to owners of the vessel with reference to:

  • Conduct a risk assessment for those activities that require work to be carried out while aloft. This should include electrical equipment isolation, radar emissions isolation, and use of personal safety equipment..

  • Ensure that when working aloft personnel wear a safety harness or other restraining/ capture equipment

  • Fit safety chains to guard rail access openings on most platforms

  • Apply non-slip finish to the forward and aft platforms
