COM10051 - Appeals: handling appeals and postponements: forms: assessment appeals in responsible CT office
The following forms are relevant to handling appeals.

| 64-4 (Ack) | Confirmation of receipt of an appeal. | 
| 64-4 | Confirmation of the amount of postponement. | 
| 64-5(1) and (2) | Used to suggest an alternative amount to be postponed. | 
| 64-5(3) | Notification of the determination of the amount of the postponement determined by the Tribunal or notified by the decision maker. | 
| AOC385 | Notification of a suspension made by the CT Unit Cumbernauld. | 
| AZ20 | Form used by the Debt Management office and the CT Unit Cumbernauld to send correspondence to the responsible CT office. The charge may or may not be suspended. | 
| C52D | Form used by the Debt Management office to send an appeal or objection to the responsible CT office. |
