Details

The YJB designed the custody exit form following feedback from practitioners and young people. It is a survey to collect the views of young people on their experiences of custody at the point of release.

Once you have completed the form you should return it to: custodyfeedbackform@yjb.gsi.gov.uk

It also contains guidance, which you should read in conjunction with section 7 of the Case Management Guidance and the national standards for youth justice services.

The form and guidance is available in English and Welsh.
