Good practice guides and evaluation reports for the LinkAge Plus pilot in Devon.
The Devon LinkAge Plus pilot had 3 elements:
- the development of a senior council putting older people in the driving seat of the later life strategy
- a self-help tool to prompt older people to think about their needs, used by the ‘MyDevon’ Customer Service Centre
- a mentoring scheme for isolated people experiencing or at risk of social exclusion
