Details

You must complete this consent form if you have parental responsibility for a child who is applying for an emergency travel document (ETD) and either or both of the following are true:

  • you will not be present at the emergency travel document interview
  • the child is travelling without anyone who has parental responsibility

Everyone with parental responsibility must complete this form. This can be:

  • parents
  • individuals named on the child’s birth certificate
  • anyone who has acquired parental responsibility in some other way such as through a court order or a parental responsibility agreement
  • a UK local authority

To complete the form, you’ll need to:

  1. Fill in the form online (or download the form and then fill it in).
  2. Print the form so that you can sign each page with a handwritten (wet ink) signature.

By signing this form, you consent to:

  • the child named on the form being issued an ETD
  • the child going on the journey proposed in the travel details on the form
