Newly qualified teachers (NQTs) employed in maintained schools, non-maintained special schools, maintained nursery schools and pupil referral units in England must, by law, complete an induction period (unless they are exempt, see annex B of the statutory guidance) after gaining qualified teacher status (QTS).

For each assessment, the induction tutor must complete these induction forms. The headteacher/principal keeps a copy and sends a copy to the appropriate body within 10 days of the NQT completing the assessment period. The original should be retained by the NQT.

These forms may be amended/added to by appropriate bodies for use in schools and colleges but none of the original fields should be removed or altered.
