If you are applying to the Horizon Shortfall Scheme (HSS) fixed sum offer permission to appeal process (FSO Appeals), you should complete this registration form with your legal representative and submit it, along with the required ID documentation, to hssappeals@businessandtrade.gov.uk.

The information provided will allow the Department for Business, Innovation, Science and Trade (BIST) to verify and confirm your eligibility for FSO Appeals.
