Use these forms if you are a forensic pathologist who is involved in an annual appraisal process.
Guidance on the appraisal process includes information on:
- when to submit supporting information to your appraiser
- what to discuss in the appraisal
- protocol for investigations or disciplinary procedures
- how to sign off an appraisal
All forensic pathologists on the Home Office Register of Forensic Pathologists have been given their recommended appraisal date for 2023 to 2024 appraisals.
These forms and guidance should also be used by forensic pathologists employed by the Department of Justice (Northern Ireland) for appraisal and revalidation purposes.
Forms AF-009 and AF-010 have been added this year.
All appraisals must be completed by 31 March 2024.
